Pull KVM fixes from Radim Krčmář:
 "KVM:
   - lock kvm_device list to prevent corruption on device creation.
  PPC:
   - split debugfs initialization from creation of the xics device to

   - prevent userspace from triggering two WARN_ON_ONCE.
  MIPS:
