xprtrdma: Refactor rpcrdma_ep_connect() and rpcrdma_ep_disconnect()
Clean up: Simplify the synopses of functions in the connect and
disconnect paths in preparation for combining the rpcrdma_ia and
struct rpcrdma_ep structures.
