This commit enables basic support for Hopper GPUs, and is intended
primarily as a base supporting Blackwell GPUs, which reuse most of
the code added here.
Advanced features such as Confidential Compute are not supported.
Beyond a few miscellaneous register moves and HW class ID plumbing,
the bulk of the changes implemented here are to support the GSP-RM
boot sequence used on Hopper/Blackwell GPUs, as well as a new page
table layout.
There should be no changes here that impact prior GPUs.

GPUs exist now with a 64-bit BAR0, which mean that BAR1 and BAR2's
indices (as passed to pci_resource_len() etc) are bumped up by one.
Modify nvkm_device.resource_addr/size() to take an enum instead of
an integer bar index, and take IORESOURCE_MEM_64 into account when
translating to the "raw" bar id.

The current code using NV90F1_CTRL_CMD_VASPACE_COPY_SERVER_RESERVED_PDES
not only requires changes to support the new page table layout used on
Hopper/Blackwell GPUs, but is also broken in that it always mirrors the
PDEs used for virtual address 0, rather than the area reserved for RM.
This works fine for the non-NVK case where the kernel has full control
of the VMM layout and things end up in the right place, but NVK puts
its kernel reserved area much higher in the address space.
Fixing the code to work at any VA is not enough as some parts of RM want
the reserved area in a specific location, and NVK would then hit other
assertions in RM instead.
Fortunately, it appears that RM never needs to allocate anything within
its reserved area for DRM clients, and the COPY_SERVER_RESERVED_PDES
control call primarily serves to allow RM to locate the root page table
when initialising a channel's instance block.
Flag VMMs allocated by the DRM driver as externally owned, and use
NV0080_CTRL_CMD_DMA_SET_PAGE_DIRECTORY to inform RM of the root page
table in a similar way to NVIDIA's UVM driver.
The COPY_SERVER_RESERVED_PDES paths are kept for the golden context
image and gr scrubber channel, where RM needs the reserved area.

to make the code easier to work with when adding support for a newer RM
version.

This is a hack around a bug exposed with the GSP code, I'm not sure
what is happening exactly, but it appears some of our flushes don't
result in proper tlb invalidation for out BAR2 and we get a BAR2
fault from GSP and it all dies.

This moves the initial effort to the latest 535 firmware.
The gsp msg structs have changed, and the message passing also.
The wpr also seems to have some struct changes.
This version of the firmware will be what we are stuck on for a while,
until we can refactor the driver and work out a better path forward.

This was cargo-culted from traces of RM when the code was written, but
we probably shouldn't be touching NV_PFB regs while GSP-RM is running.
From traces, it looks like NVIDIA dropped this sometime between 510.54
and 515.48.07, so I guess we can too.

The new VM_BIND UAPI uses the DRM GPU VA manager to manage the VA space.
Hence, we a need a way to manipulate the MMUs page tables without going
through the internal range allocator implemented by nvkm/vmm.
This patch adds a raw interface for nvkm/vmm to pass the resposibility
for managing the address space and the corresponding map/unmap/sparse
operations to the upper layers.

Exposes a bunch of the new features that became possible as a result
of the earlier commits. DRM will build on this in the future to add
support for features such as SCG ("async compute") and multi-device
rendering, as part of the work necessary to be able to write a half-
decent vulkan driver - finally.
For the moment, this just crudely ports DRM to the API changes.
- channel class interfaces now the same for all HW classes
- channel group class exposed (SCG)
- channel runqueue selector exposed (SCG)
- channel sub-device id control exposed (multi-device rendering)
- channel names in logging will reflect creating process, not fd owner
- explicit USERD allocation required by VOLTA_CHANNEL_GPFIFO_A and newer
- drm is smarter about determining the appropriate channel class to use

Some NVIDIA GPUs do not support direct atomic access to system memory via
PCIe. Instead this must be emulated by granting the GPU exclusive access
to the memory. This is achieved by replacing CPU page table entries with
special swap entries that fault on userspace access.
The driver then grants the GPU permission to update the page undergoing
atomic access via the GPU page tables. When CPU access to the page is
required a CPU fault is raised which calls into the device driver via MMU
notifiers to revoke the atomic access. The original page table entries
are then restored allowing CPU access to proceed.

When migrating a range of system memory to device private memory, some of
the pages in the address range may not be migrating. In this case, the non
migrating pages won't have a new GPU MMU entry to store but the
nvif_object_ioctl() NVIF_VMM_V0_PFNMAP method doesn't check the input and
stores a bad valid GPU page table entry.
Fix this by skipping the invalid input PTEs when updating the GPU page
tables.

setting the hardware specific GPU page table entries for 2MB sized
pages. Fix this by adding functions to set and clear PD0 GPU page
table entries.

Turing introduced a new simplified page kind
scheme, reducing the number of possible page
kinds from 256 to 16. It also is the first
NVIDIA GPU in which the highest possible page
kind value is not reserved as an "invalid" page
kind.
To address this, the invalid page kind is made
an explicit property of the MMU HAL, and a new
table of page kinds is added to the tu102 MMU
HAL.
One hardware change not addressed here is that
0x00 is technically no longer a supported page
kind, and pitch surfaces are instead intended to
share the block-linear generic page kind 0x06.
However, because that will be a rather invasive
change to nouveau and 0x00 still works fine in
practice on Turing hardware, addressing this new
behavior is deferred.

If the BAR initialization failed it may leave the vmm structure in an
unitialized state, leading to a null-pointer-dereference when the vmm is
dereferenced during teardown.

Some GPU units are capable of supporting "replayable" page faults, where
the execution unit will wait for SW to fixup GPU page tables rather than
triggering a channel-fatal fault.
This feature isn't useful (it's harmful, even) unless something like HMM
is being used to manage events appearing in the replayable fault buffer,
so, it's disabled by default.
This commit allows a client to request it be enabled.

Host methods exist to do at least some of what we need, but we are not
currently pushing replay/cancels through a channel like UVM does as it's
not clear whether it's necessary in our case (UVM also updates PTEs with
the GPU).
UVM also pushes a software method for fault cancels on Pascal, seemingly
because the host methods don't appear to be sufficient. If/when we want
to push the replay/cancel on the GPU, we can re-purpose the cancellation
code here to implement that swmthd.

NVKM is currently responsible for managing the allocation of a client's
GPU address-space, but there's various use-cases (ie. HMM address-space
mirroring) where giving a client more direct control is desirable.
This commit allows for a VMM to be created where the area allocated for
NVKM is limited to a client-specified window, the remainder of address-
space is controlled directly by the client.
Leaving a window is necessary to support various internal requirements,
but also to support existing allocation interfaces as not all of the HW
is capable of working with a HMM allocation.
